    PRE-TRAINING SURVERY

    COURSE OR PACKAGE SELECTION

    ​For Group Course Only

    - Pro Rider = Smart Rider + Master Rider
    - We will arrange the earliest avaiable course dates for you.
    - We will email to you to confirm the dates.
    - E-mail us if you want to be a specified course dates.
    - There is no guaranty for any course dates you choose.
    - If you want to pre-register the course in the later dates, please provide the Course Number in the following. We will try our best to arrange.

    ​For Private Training Only

    -  Please provide the date and time if you have booked your MST (at Burnaby ICBC)
    - We suggest that we booked for you, so that we can guarantee the availability of the school motorcycle. 
    - Please consult with us before your registered if you want to do your MST at any other locations.

    PAYMENT METHOD

    We will charge a one hundred dollar ($100) deposit at the time of registration. The tuition fee must be paid in full 10 days prior to the start date of the course or package. All fees paid, except the security deposit, are conditionally refundable but non-transferable. Please refer to the Terms and Service Policy for more details.​


    ​When sending e-transfer, please indicate your full name and DL number in the note.​      If paid by cash, please email us arrange an appointment for the payment.  ​

    SECURITY DEPOSIT

    • A security deposit by credit card is required for registration, even if you choose to pay by E-Transfer or cash.
    • If you use a credit card, it will not be charged unless there is damage to the motorcycle or equipment.
    • If you pay the security deposit in cash, it will be fully refunded after the completion of the course.

    CREDIT CARD AUTHORIZATION

    Whether you choose to pay for the course or package with a credit card, or use a credit card for the security deposit, please enter your information below. 
    Note: The billing address and phone number must be the same as your credit card statement
    I agree and authorize KamenRider School to charge the above credit card for the following, if necessary:
    1. Group Course or Private One-on-One fee
    2. Deposit at Registration
    3. Any applicable taxes
    4. Additional training fees or any outstanding balances
    5. Security deposit or the final cost of repair/replacement (including parts and labor) for any damage to the school’s motorcycle and/or equipment
    6. Administration fee
    By checking the box below, I declare and certify that I am the primary cardholder of the credit card specified above, and I have the legal right to authorize KamenRider School or its employees to incur charges on my behalf. This authorization will automatically expire upon the completion of my registered course, session, Road Test, and/or once I have fully settled all charges, payments, fees, and/or damage costs with KamenRider School

    WAIVER OF LIABILITY

    I hereby release KamenRider School, its officers, instructors, and any related agency sponsoring KamenRider School from all responsibility for property damage, bodily injury, liability, costs, expenses, and claims of every nature and kind, however arising, as a result of my participation or behavior in any of the training courses, sessions, or lessons conducted by KamenRider School or its instructors. I also agree to hold harmless KamenRider School, its officers, instructors, and any sponsoring agency from all claims and rights of action that may arise through my participation in any training session. I acknowledge the risks involved in learning to ride a motorcycle or scooter, including the possibility of injury.
    I have read, understand, and agree to all terms and conditions outlined in the Terms of Service Policy and the Privacy & Security Policy. I certify that I am over 19 years of age and am signing the above liability release of my own free will. (If you are under the age of 19, your parent or legal guardian must sign on your behalf before the course begins.)
